Directions: Complete these six areas to assess what you already do to take care of yourself and to establish new self-care goals. If you need support in implementing goals that are related to your work as a professional or your program, discuss this with a coach, trainer, or administrator. For each of the six areas identify:
1. Psychological(Examples: Take time for yourself; disconnect from electronic devices; journal; pursue new interests; learn new skills; access psychotherapy; life coaching, or counseling support if needed.) 2. Physical(Examples: Eat regular and healthy meals; maintain good sleep habits; exercise regularly; get regular medical check-ups.) 3. Personal(Examples: Have healthy relationships; make time for family and friends; schedule dates with partner or spouse; ask for support from family and friends.) 4. Emotional(Examples: Engage in positive activities; acknowledge own accomplishments; express emotions in a healthy way.) 5. Spiritual(Examples: Read inspirational literature; self-reflect; spend time in nature; meditate; explore spiritual connections.) 6. Professional(Examples: Pursue meaningful work; maintain work-life balance; have positive relationships with coworkers; engage in good time-management skills.)
