Think about a child you have never met before. Make a list of all the things you would like to know about the child and learn from his or her family. Create a questionnaire that would help you capture this information. If you currently use a Family Questionnaire, compare your questions and see if any improvements could be made. Share your responses with your trainer, coach or family care administrator.
